$75,000 to support the House to Home program; a mobile unit that will deliver in-home, enhanced telehealth, and wraparound services directly to children enrolled in GCH’s Complex Care Program allowing them to have virtual appointments with their providers while eliminating the barrier of transportation.
$100,000 to maintain, improve, and expand transportation services for the Rochester area by implementing internal education to certify drivers, replacing vehicles nearing end-of-life, and upgrading technology to improve communications.
